Here is how we will use RCA to improve your processes

  • We will define the problem by clearly articulating the problem or issue that needs to be addressed. We will help you write a problem statement that describes the problem, its impact, and its scope.

  • We will gather data by collecting relevant data and information about the problem, including incident reports, customer feedback, process documentation, and performance metrics. We will help you use various sources and methods, such as interviews, surveys, observations, or records, to gather and organize the data.

  • We will identify contributing factors by using techniques like brainstorming, interviews, or surveys to identify potential contributing factors to the problem. We will help you involve and engage the relevant stakeholders, such as employees, customers, or suppliers, to get their input and perspectives.

  • We will analyze data by applying qualitative and quantitative analysis techniques to examine the data and identify patterns, trends, or anomalies. We will help you use various tools and techniques, such as histograms, Pareto charts, run charts, and scatter plots, to display and analyze the data.

  • We will use cause-and-effect tools by utilizing tools such as fishbone diagrams (Ishikawa), 5 Whys, or fault tree analysis to visually map out the potential root causes and their relationships. We will help you use these tools to explore and test the possible causes and effects of the problem.

  • We will prioritize root causes by evaluating and prioritizing the identified root causes based on their significance and impact on the problem. We will help you use various tools and techniques, such as matrix diagram, risk assessment, or cost-benefit analysis, to rank and select the most critical root causes.

  • We will verify root causes by validating the potential root causes through data analysis, expert opinions, or experimentation. We will help you use various tools and techniques, such as control chart, hypothesis testing, or pilot testing, to confirm or reject the root causes.

  • We will develop corrective actions by generating possible corrective actions or solutions to address the identified root causes. We will help you use various tools and techniques, such as brainstorming, affinity diagram, nominal group technique, or matrix diagram, to create and evaluate the possible actions.

  • We will implement corrective actions by putting the selected corrective actions into practice, making sure to communicate and coordinate with relevant stakeholders.   • We will monitor and evaluate by continuously monitoring the implemented actions to assess their effectiveness and make adjustments if needed. We will help you use various tools and techniques, such as control chart, run chart, or feedback survey, to measure and report the outcomes and impact of the actions.

  • We will document the analysis process by documenting the entire root cause analysis process, including the problem statement, data collected, analysis techniques used, identified root causes, and implemented actions. We will help you use various tools and techniques, such as A3 report, lessons learned template, or best practices repository, to document and share the process and results.
